The Supervised Housing program, Delta House, offers an inclusive array of community based rehabilitative services for persons with mental illness who need supervised services to prepare them to live independently within their community. Our objective is to provide clients with the opportunity to acquire, maintain, or improve the skills necessary for ultimately living independently in the community through a supervised and structured group living arrangement. The house has staff available to clients 24/7 on site. The home operates under a level system that allows clients an opportunity to progressively earn increased levels of independence as new skills are acquired.

Clients living at Delta House require a well-coordinated and integrated package of services provided over an extended period of time for them to live successfully in the community. These services include but are not limited to the following:

• Assessment and Service Planning: Determining the array of services required by the individual; identifying the services or programs that will best meet the needs identified in the service needs evaluation and which will be/is incorporated into the ITP.
• Referrals and Linkage/Case Management: Assisting individuals in accessing items needed to set up housekeeping; linkage with resources and services; providing personal support and assistance in gaining access to other mental health treatment and rehabilitation services, vocational training services, educational services, legal services, employment services, leisure recreation, spiritual, social activities, cultural activities and self-help groups. Accessing and providing training in obtaining medical services emergency and non-emergency.
• Counseling: Providing supportive counseling and problem solving.
• Advocacy: Advocating on behalf of the individual; providing appropriate information to family members in a crisis situation or when the client has deemed certain information can be released.
• Support: Ongoing assistance will be provided to the individual in developing natural community supports, fostering relationships with non-paid persons in the community such as neighbors, and volunteers.
• Assistance: Skills development necessary for individuals to live as independently as possible will provided through assisting the individual in activities of daily living through skills training, such as self-care and personal hygiene, cooking, home improvement, behavior management/anger control, dating and sexual appropriateness, money management and acquisition of assistive devices.
• Transportation: Providing and assisting with transportation, teaching the individual to use and access public transportation or provide transportation to medical/psychiatric services or other needed services.
• Crisis: Provide intervention and support in times of crisis, including 24-hour crisis response; providing on-going support following a crisis.
• Representative Payee Services: Providing the needed service of financial payee for these deemed appropriate by Social Security.
• Budgeting: Providing a written monthly budget for those clients receiving representative payee services.
• Recreation Activities: Providing recreational activities that will enable individuals to interact within community based activities. Provide training in leisure time skills. Programs may be developed with other non-supportive housing consumers within the community.
• Medication Monitoring/Training: Monitoring to ensure that individuals obtain the necessary medicine, are instructed in self-administering medication; monitoring to be sure the individual is taking medication.
• Facilitation of consumer run groups and activities.
• Ongoing support/services as the individual explores changes in their living arrangements. Outreach support for an identified length of time to clients who move to more independent living arrangements.
• Instruction in proper nutrition, meal preparation, and menu planning.
• Community Skills such as, use of public and private transportation, use of public facilities such as stores, restaurants, public restrooms, theaters, public behavior, use of community services, use of public, financial and social service agencies.


Living Arrangements:
Each person served in the residential setting will have personal space that respects privacy, promotes personal security and safety. There will be an opportunity for expression of choice by persons served as to the room they are assigned and, whenever possible, the choice of roommates.

Admission Criteria includes being at least 18 years old, having a primary diagnosis of a major mental illness, having independent physical mobility, and the capability of administering own medications. Clients are required to make Delta Center Inc. their representative payee and must attend a psychosocial rehabilitation program during the week. Residents are also required to contribute a portion of their income toward room and board. Clients who have extensive medical conditions, legal problems, are prone to physical aggressiveness, or those who are unwilling to participate in the program, will not be considered for admission.
